Duty of Candour NUL report 2024/25
All health and social care services in Scotland have a statutory duty of candour. This legal duty ensures that when things go wrong and an incident causes harm, the people affected are informed about what has happened, receive an apology, and are supported appropriately. It also ensures that organisations learn from incidents to improve services in the future.
As part of this duty, we are required to publish an annual report outlining how we have met our duty of candour responsibilities during the reporting period. This report describes how our service has operated the duty of candour between 1 April 2024 and 31 March 2025.
1. Incidents to which the duty of candour applied
During this reporting period, there were no incidents that met the criteria for the duty of candour to apply.
2. Policies and procedures relating to the Duty of Candour
Where an incident occurs that triggers the duty of candour, staff are required to report this to the service manager. The service manager is responsible for ensuring that the duty of candour procedure is followed in line with legislation and guidance. All relevant incidents are recorded and, where required, reported to the Care Inspectorate.
When an incident occurs, the manager and staff involved carry out a learning review. This process allows those involved to reflect on what happened, identify any learning, and make improvements to reduce the risk of similar incidents happening in the future.
All new staff receive information about the duty of candour as part of their induction. We recognise that serious incidents can be distressing for staff as well as for the people who use our services and their families. Appropriate occupational welfare support is available to staff who may be affected.
Where children, individuals, or families are affected by an incident relating to the duty of candour, we have arrangements in place to provide welfare and support as required.
